I was very impressed with my appointment and the way I was treated, now I understand why patience come from all over the world to see you.Can you believe I have suffered with this for 42 years and so meny course’s of antibiotics, It is just heart braking for me. So the treatment, I started to improve almost immediately on Monday 21, night sweats, stopped, urgency to pass urine stopped, burning inside stopped, out of ten 9.5. night urination stopped, sleeping at last all throw the night. Good evening Prof. Guercini, as agreed, I inform you about the evolution of the therapy started on 12/29/2018.

The symptoms have been greatly reduced for about ten days and for a few days they were almost completely absent. Sometimes I still feel modest discomfort after urinating and in conditions of particular stress sometimes the sense of “contraction” on the lower part of the penis also reappears. However, as already mentioned these discomforts have greatly decreased both in frequency and intensity…. so I am very confident and I sincerely think that we are on the right path…. It’s been 11 years now since we last met. You had treated me for a long standing Chlamydia infection.  I’m Monty Anderson from Windermere, British Columbia, Canada.  My bladder has been atonic because of that infection, so I have to empty it more frequently. Also, I have had mild prostatic pain in the mornings.  Not too bad after what I had been through for the previous 15 years. Thank you for relief I’ve had these past 11 years! Now, things are changing quickly for me.  I’m in a lot of discomfort from BPH and it is difficult to urinate.  My PSA levels are low, but my prostate is 120cc.  I feel like there is a large ball inside me, ready to explode. I take Alfuzosin 10 mg to relax the muscles and I’ve tried Dutasteride 0.5 mg for about a month with no relief. I’ve read articles that say you need more dihydrotestosterone and less estrogen to shrink the prostate.   I’m not sure which way to go.  Can you offer some suggestions for shrinking the prostate?  Or do I need a surgical intervention at this point?

How are you? I am writing to tell you that for now I feel very well.

As for the issue of erections, the truth is that they are durable and firm. Yes, sometimes, although not very frequent, I still notice a few small pangs in the area of the perineum, but not much. Is this normal?

Every time I empty my bladder completely, and the urine comes out strongly. I’m pretty happy in general.

I still do not abuse spices and spicy foods, as well as drinking alcohol. I do not usually drink a lot, but every once in a while I drink some beer, and I do not have any discomfort when I want to urinate, it’s fantastic!

Dear Prof. Guercini, thanks to your therapy I continue to improve. Your effectiveness and speed amaze me after having taken so many drugs in vain. The spontaneous pains have practically disappeared. There remains tenderness upon palpation of the left testicle with a small trigger point at the base of the cord. The right is practically back to normal. I continue with a scrupulous diet which I intend to continue for at least a year. I am about to leave for the Red Sea where I will do calm dives within 30 metres. Last year I had no problems despite Rixxxxx and Lioxxxx and it seems to me that Laroxxxx at this dosage has a decidedly lower impact. I will bend over backwards to avoid pepper and spices.

Dear Professor
The therapy is now in its 20th day and I have practically returned to a normal life. The acute pains disappeared already with the first applications of hot cones (water is needed at 60 degrees) and the anal sphincter regained normal elasticity (I was terribly contracted and the cones entered with difficulty).
Every now and then I have very slight burning sensations after urination if I have retained urine for a long time but then they disappear quickly. The nocturia is no longer painful and I go to the bathroom around 5.30-6 and sometimes I don’t even wake up until I wake up. I am tolerating the Lioxxxx and Rixxx very well, even managing to run for 5-6 km again.  Sexual activity returned to normal without pain upon ejaculation or after intercourse with normal libido. There remains pain when palpating the epididymal cysts, especially on the left, with occasional painful twinges in the left funiculus and I have started therapy with seleparin in case there is any thrombosis.
I don’t know how to thank you for getting me out of a tunnel of pain and depression that lasted six months amidst the indifference or ignorance of so many colleagues.  I will certainly remember you when my patients have urological problems.
I will inform you of the next update in 20 days.

Dear Professor,

I’m sure he remembers me. The last time I wrote to her was around this time last year. I had to leave for Bologna for yet another hip operation. I have to say that everything has happened to me. When I went for pre-hospitalization, I fell in the hotel and broke my wrist. Surgery postponed. Finally  they operated on me but without doing great things, only the replacement of the polyethylene. given that if they touched the prosthesis it was not possible to fix it again due to a notable lack of periprosthetic bone.

And what happened to my cystitis in the meantime???

For this it is a letter of affectionate thanks. After six months of therapy, especially with the antidepressant, I stopped getting up 3-4 times a night, visiting all the bathrooms and I stopped wearing that terrible thing, the sanitary pad for urinary leaks. Despite Parkinson’s I have made progress without cystitis I feel like a normal person. I hope to be able to send you the attachment to demonstrate my well-being. THANK YOU professor, thank you very much. She gave me back my dignity… because cystitis for 13 years also destroys your morale.

More than a visit, I would like to share what was a rare medical diagnostic experience.

I was….finally I can use the past suffering from chronic bacterial prostatitis!!! I have the double fortune of being able to talk about Prof. Federico both as a colleague and as a patient. We have known each other for almost twelve years, during which he treated me with dedication, competence and effectiveness that I had never encountered in other urologists (and God only knows how many I consulted before him!). After years of suffering caused by a very common but disabling disease like few others (namely prostatitis), Prof. Federico, against everyone’s opinion except mine, agreed (and for this reason I consider him more than a friend) to undergo a radical prostatectomy. 5 stars are not enough to talk about Prof. Guercini and evaluate his work, a clear and completely starry August sky is needed! Met in 2000 for urological problems of my son who is now 44 years old, he treated him and allowed him to have two splendid children. In 2014, after days of terrible renal colic suffered by my husband due to a stone in the urethra, with a treatment the Professor helped resolve the problem definitively. After 22 years of continuous cystitis and difficulties in urination, caused by an overcorrection of the bladder carried out by another doctor during an operation for “Hysterectomy for recurrent uterine fibromatosis”, on 09/28/2016 I was operated on by the Professor and I solved my problems brilliantly. Today I was reborn, I went back to living without the terror that has pervaded me all these years, fearing that the infection could also spread to my kidneys considering the fact that I am highly allergic to drugs! There is not enough affection, esteem, gratitude to thank a man of science like Prof. Guercini who looks his patients in the eyes and takes care of them as he would his own children. Dear Professor
The therapy is now in its 20th day and I have practically returned to a normal life. The acute pains disappeared already with the first applications of hot cones (water is needed at 60 degrees) and the anal sphincter regained normal elasticity (I was terribly contracted and the cones entered with difficulty). Every now and then I have very slight burning sensations after urination if I have retained urine for a long time but then they disappear quickly. The nocturia is no longer painful and I go to the bathroom around 5.30-6 and sometimes I don’t even wake up until I wake up. I am tolerating the Lioxxxx and Rixxx very well, even managing to run for 5-6 km again. Sexual activity returned to normal without pain upon ejaculation or after intercourse with normal libido. There remains pain when palpating the epididymal cysts, especially on the left with occasional painful twinges in the left funiculus and I have started therapy with seleparin in case there is any thrombosis. I don’t know how to thank you for getting me out of a tunnel of pain and depression that lasted six months amidst the indifference or ignorance of so many colleagues.
I will certainly remember you when my patients have urological problems.
I will inform you of the next update in 20 days.

I’m sure he remembers me. The last time I wrote to her was around this time last year. I had to leave for Bologna for yet another hip operation. I have to say that everything has happened to me. When I went for pre-hospitalization, I fell in the hotel and broke my wrist. Surgery postponed. Finally they operated on me but without doing much, just replacing the polyethylene. given that if they touched the prosthesis it was not possible to fix it again due to a notable lack of periprosthetic bone.

And what happened to my cystitis in the meantime???

For this it is a letter of affectionate thanks. After six months of therapy, especially with the antidepressant, I stopped getting up 3-4 times a night, visiting all the bathrooms and I stopped wearing that terrible thing, the sanitary pad for urinary leaks. Despite Parkinson’s I have made progress without cystitis I feel like a normal person. I hope to be able to send you the attachment to demonstrate my well-being. THANK YOU professor, thank you very much. She gave me back my dignity… because cystitis for 13 years also destroys your morale.

Postoperative anal prolapse, therefore: OP by Longo (STARR procedure)

Over the years I had ongoing recurrent Prostatitis, always bacterial cultures positive: in the beginning, only Enterococcus faecalis, then plus Klebsiella pneumoniae (3MRGN), then also E. coli (3MRGN). Most multiple bacteria positive – long time antibitiocs with Ciprofloxacin, Amoxicillin, but also prolonged course with Meropenem iv and Linezolid iv didn’t lead to eradication, but always to recurrence

09/2016 MRI pelvis [Houston]
intramural fistula tract, which penetrates from the rectal wall anteriolaterally to a small collection of the left rectoprostatic angle (however depending on who read the MRI – this was the official report – other people said they couldn’t see anything in this MRI !)

I moved to Germany for further treatment or diagnosis of a potential fistula

02/2017 [Prof. Matzel]
Rectal window (small submucuos abscess) and OTSC clip in the rectum

Prof. Matzel is a very experienced colorectal surgeon, he could see this small abscess, but couldnt see any fistula. He doubts that there is a fistula.

Several cystoscopies by two different urologist:

It shows a fistula opening (prostatate duct ?) from the prostatic urethra right dorsal, close to the Colliculus seminalis. Injection with contrast agent and also direct probing – it ends blind in the prostate, although the direction is towards the previous submucous abscess in the rectum. A rectoscopy done at the same time doesn’t show any contrast agent in the rectum (methylenblue), at the same time, there was no air (when the rectum was filled with air) seen in the bladder. All concluded there is no fistula.

On rethrograde urethrogram this duct is also identifiable, but only during high pressure of voiding – reflux into, not during normal voiding.

On videourodynamic probably minor infravesical obstruction, no reflux into prostate.

Ultrasounds and all other investigations seem normal. My prostate is rather small 30cc.

Since the methylenblue cystoscopy my symptoms have significantly aggrevated (3 weeks now) with pain in my right testicle, perineal and suprapubic pain. My recent cultures showed originally the always present E. coli (3MRGN), Klebsiella (3MRGN) and Enterococcus, however after this cystoscopy my cultures became negative. Currently I am on a two weeks course at home with Imipinem with no change in the symptomatic. Especially any voiding seems to aggravate my symptomatic.

My prostatitis evaluation scores are consistently very high, and I do apologize again for this direct approach, but I am really desperate to find some sort of improvement. Of course, I would be more than happy to fly to Rome to see you as well.

It has now been six months since I attended your clinic for the last injection. I have noticed an obvious improvement in the prostatitis symptoms since the treatment. Overall I would say that my symptoms have improved by about 60-65% since I received the injection treatment. The feeling of inflammation around the prostate is less severe and I have also noticed that I have stronger erections than before the treatment- which I was not expecting.

The results after the first 10 days are amazing!!!! after just one day of therapy I had an erection as it was about two years ago with the right rigidity and durability (not verified with reports, but I really think there are no problems). I seem to have returned to when I was 20 years old. I am also able to have an erection at any time of the day with a little mechanical stimulation. However, poor ejaculatory capacity and low sperm volume persist (this summer it had improved due to taking arginine prescribed by an andrologist); Approximately 1-1.5 ml with abstinence of 2-3 days. However, libido is still low. I am very happy and with the trust I give you I hope that the results will last over time. Side effects: negligible. some insomnia, often some palpitations or brief tachycardia and dry mouth due to taking Toviaz.

I have the pleasure of communicating to you, as you requested during the visit on 11/23/2010, the first impressions and results of the therapy prescribed by you which I started on 11/27/2010. FINALLY! And I have the pleasure of saying FINALLY, after months I can say that I have been feeling decidedly better for a few days. After an initial period characterized by a sense of torpor, dizziness, which disappeared after a few days, that annoying, insistent and stinging pain in the groin, the heaviness, and the chronic resentment in the right testicle has also significantly regressed. Even those pains – burning in the buttock and consequent radiation to the right leg – seem to have disappeared. Another improvement concerns the problem of my congenital sclerosis of the bladder neck. I seem to urinate more frequently, and also with more ease. This improvement was evident especially in the first days of therapy, then as the days went by it seems to have diminished.
